Eligible CountyCare members can receive fresh food boxes or ready-to-eat meals delivered to their door at no cost. You also get full access to the Foodsmart app and your own nutrition expert—a registered dietitian—to help you reach your health goals. Starting March 1, 2026, members who are pregnant with gestational diabetes or gestational hypertension or managing both diabetes and high blood pressure and uncontrolled illness as identified by CountyCare can receive home delivered meals.

Talk to a registered dietitian by phone or video to create a plan that fits your life. Starting March 1, 2026, qualifying members can access up to 6 visits per year.

  • Pregnant persons who develop gestational diabetes or pregnancy induced hypertension.

How do I know if I qualify for food deliveries?

Starting March 1, 2026, CountyCare has updated its benefits to focus food deliveries on members who are pregnant with gestational diabetes or gestational hypertension and those managing both diabetes and high blood pressure and uncontrolled illness as identified by CountyCare.

What if I don't meet those specific health criteria?

You still have full, free access to the Foodsmart app! You can use the NutriQuiz, meal planners, grocery price-comparison tools, and recipes to help you stay on track with your health goals at any time.

How will this program work with my other providers and care manager team?

Working with a nutrition coach is a wonderful addition to any care management team. We'll work to understand your current medications, practices and goals, then develop a nutrition plan that can fit easily into your routine.
