Do you want an extra $50 for other holiday groceries?
Sign up for an initial visit with a Foodsmart nutrition coach. You will receive $50 in grocery money after completing your first visit.
Follow these simple steps and book your first visit:
- Visit Foodsmart.com/schedule to book your audio or virtual visit with a Chicago nutrition coach at no cost to you.
- Follow the prompts to choose a date & time that works for you.
- Locate your HealthChoice Illinois CountyCare member ID card and enter your member ID when prompted.
- Complete your appointment request!
- During your visit ask your dietitian about our custom nutrition programs to jumpstart your progress.
- After you complete your first visit you will receive your $50 in grocery money.
Frequently Asked Questions
How can I guarantee that I receive a turkey and sides?
We are taking up to 100 reservations per event. To reserve a turkey and sides, click this link. If reservations are sold out, there are 100 turkeys and sides available at each event on a first-come first-served basis.
Will there be reheat instructions for the turkey and sides?
Yes, you will be provided with storage and reheat instructions for your holiday meal.
If I have food allergies, can I request a special meal?
Unfortunately we are not able to make special accommodations for allergies or food preferences. All meals come with a 10-12 lb turkey, candied yams, southern green beans, and balsamic & parmesan crusted brussel sprouts
If my plans change and I can’t come pick up my turkey and sides, can you ship it to me?
The turkey + sides are prepared fresh and cannot be shipped. If you reserve a meal and are not able to pick it up we will donate it to another community member at the end of the event.
I can’t come to any of the events, is there another way I can get help with food this holiday season?
CountyCare members can receive $50 in grocery money after completing their initial visit with a Foodsmart Dietitian.

We offer CountyCare members a simple solution to managing their health and reaching their nutrition goals.Foodsmart connects CountyCare HealthChoice Illinois members with their own registered dietitian, a nutrition expert who is licensed to support members with common medical conditions, like diabetes, cancer, heart conditions and obesity, through nutrition and healthy choices. Foodsmart's digital app helps members with meal planning, shopping on a budget, and food delivery.
